Funding physician networks

 

With this funding, the German Foundation for the chronically ill aims to support the establishment of regional, IT-supported physicians networks with the help of start-up funding for a year.

The establishment of medical care networks should help to overcome between disciplines and sectors and to ensure a structured, coordinated and guideline-oriented care process.

Partner in a care network should develop common treatment concepts so that the diagnosis and treatment steps are coordinated and coherently build on one another.

 

 


                

Building physician networks: 2nd round of funding 9/19-9/20

In this second round, 7 projects are funded:

  • Heartfailure-Network-WESTPFALZ-KLINIKUM, Kaiserslautern
  • Telemonitoring Rheinland-Pfalz, Koblenz
  • Discharge HI Nurse im HI Netzwerk Würzburger Weg, Würzburg
  • HeartFailure-Network-München Klinik Neuperlach, München
