ACHT - Adipositas Care and Health Therapy - is a digitally-supported, structured, cross-sectoral and close to home program for the postoperative care of patients following obesity surgery. The aim is to ensure the long-term success of therapy after surgery. ACHT was selected by the Federal Joint Commission (G-BA) for support under the Innovation Fund. The project started in July 2019 and will end in December 2022.
In the EU-funded project "PASSION-HF: the virtual doctor Abby is being developed in cooperation with heart failure patients, relatives and medical service providers. With the help of a self-learning decision-making support system, Abby is meant to help people with heart failure in their daily management of the disease as well as in their treatment decisions.
The aim of SimPat is to develop, implement and evaluate an IT-based case management tool for multimorbid patients with dementia. SimPat connects the actors involved in the care and treatment of people with dementia after a traumatic fall: relatives, caregivers, general practitioners and hospital-based physicians.
